Before the defense, the student should submit to the Student Service Center (COS) an application for the following: a diploma of graduation together with a diploma supplement and two copies of the diploma, including one copy in a foreign language.

The diploma copy may be issued in translation into one of the following foreign languages: English, French, Spanish, German, or Russian.
A copy of the supplement may be issued in English translation.

If you want to receive your diploma in English, you need to fill in this application and make the appropriate payment.

The documents: diploma, diploma supplement, copies of the diploma and other documents from the course of studies at the written request of the student or graduate may be sent by mail to the indicated address with return receipt.

A confirmation of payment for registered mail with return receipt must be attached to the application.

The fee amount is based on the rates in the signed agreement between the University of Łódź and the current postal operator - currently Poczta Polska.

General application form

Resumption of studies may take place for a particular semester. Applications for resumption of studies should be submitted to the Student Service Center (COS) at the beginning of the semester (i.e. in February-March or September-October). Along with the application to resume studies, you must submit an application to join the seminar group with the opinion of the supervisor.

After resuming studies, it is necessary to supplement the missing credits from the semester or year (from which you were removed) and to pay for each failed subject a fee to your individual virtual bank account. It is also necessary to supplement the curricular differences - learning is then extended by an additional semester or a year. The student does not bear additional costs due to supplementing the program differences.


Application for resumption of studies

Candidates for studies who do not want to take up studies after recruitment (they want to resign from studies) and have not yet signed the pledge or agreement with the university should submit a written declaration of not taking up studies at the Student Service Center (COS) - you can use the form below.

Students who want to resign from studies after starting studies (i.e. after signing the pledge and agreement with the university) should submit a written resignation to the Student Service Center (COS).

A student is also obliged to electronically fill in a clearance form in USOS where they specify the reasons for the resignation. If you may not submit the resignation in person, it can be sent by post.

A student may appeal from every administrative decision issued by the Dean of the Faculty. The appeals are addressed to the Rector of the University of Lodz and have to be submitted within 14 days from the decision delivery. 

If a student does not wish to appeal and wants the decision to come into effect sooner, they may submit an application form to waive their right to appeal in Students Service Centre. In such case, the decision comes into effect with the date of application submission.

If for some reasons you cannot hand in or pick up some of your documents from the Students’ Office you have to make sure that your relative or friend has an authorization to do it for you.

Students in diffucult financial situation might apply for a reduction in the tuition fee. A student needs to provide documents to prove the situation. 

A student of the University of Lodz going on an Erasmus+ study stay in a different university can apply for a 50% reduction in the tuition fee. The application should be submitted in the Students Service Centre.

Statement

A student who has submitted a resignation form or has not undertaken studies, can apply for partial/full reimbursement of the tuition fee. The application has to be submitted in Student Services Centre. 

In case of the resignation done during the study period, the reimbursed fee will be reduced according to the number of weeks studied.

Application for IOS

 

If you applied for IOS for reasons other than going to Erasmus - once you receive permission for IOS, please download this form, discuss with the teachers about the method of obtaining credits, get their signatures and bring it completed to Students` Office.

IOS card

Individually arranged study schedule and programme (IPS) enables outstanding students to enrich their knowledge within the studied programme or similar programmes. It may also enable the student to change chosen courses or shorten the study period.

IPS is available for students of:

  • second year students of I cycle studies
  • second semester of II cycle studies

who achieved a grade average not lower than 4.0 during the realised course of studies.

The decision on granting IPS to a student is made by the Vice-dean following student’s application. A student wishing to apply for IPS should submit an application form along with the IPS schedule approved by IPS tutor appointed by the Dean.

A student who fails to fulfil IPS requirements might be asked to continue their studies according to the regular schedule.  The student may also resign from continuing IPS on their request. The Vice-dean specifies the deadlines for fulfilling the missing requirements of the regular schedule.

You may apply for taking course credit test in front of the board (when you have failed the taking course credit test) if there were any circumstances that might point to improper administration of the course credit test. The application form has to be submitted not later than 7 days of the announcement of course credit test scores. The course credit test has to take place not later than within 10 days of the Dean’s decision.

You may apply for an exam in front of the board (when you have failed the exam twice) if there were any circumstances that might point to improper administration of the exam. The application form has to be submitted not later than 7 days of the announcement of exam scores. The exam has to take place not later than within 10 days of the Dean’s decision.

If you, for important reasons missed a deadline during examination session, use this application to ask the Dean to have some more time to obtain your credit.

Application for prolonging the deadline to obtain course credit

You can apply for conditional semester credit if failed one or two courses in a given academic year (no more than two per year). You will retake these subjects in the following academic year. The application has to be handed within 7 days from the end of the winter re-take session (after the winter semester) and by 30.09. (after the summer semester).

You will have to pay per each course that you will retake in the following academic year (winter semester courses in the following winter semester and summer semester courses in the following summer semester).  If there are any differences in the number of ECTS credits awarded during the condition credit, You will be to obliged to supplement the missing ECTS credits by completing the elective course.

In the year time you can have only two subjects failed.

Subjects from the penultimate semester of study, subject to the condition, may be pursued in the last semester of studies with the consent of the teacher conducting the studies.

If you failed more than two courses and if you are not the first year student, you may apply for a repetition of a semester or a year. The application has to be handed within 7 days from the end of the winter re-take session (after the winter semester) and by 30.09. (after the summer semester).

You will have to pay  per each course. You should pay by 15th of October in the winter semester and by 25th of February in the summer semester.

Subjects from the winter semester are passed in the winter semester, and subjects from the summer semester are passed in the summer semester.

A student who is sent to repeat a semester shall be entitled to pass a maximum of two courses included in the study plan of a higher semester, however, "advance" shall not include:
-subjects included in the prerequisite,
-subjects from the pool of elective courses,
-seminars,
-subjects from specialization and elective modules.
Failure to pass a subject included in "advance" requires passing this subject in a conditional mode, therefore the student who has one unsettled conditional entry may only apply for "advancement" to one subject, whereas the student who has two unsettled conditional entries cannot apply for "advance".

If you want to do some courses from the subsequent years during your current year of study, you have to fill in this form and submit it in the Students’ Office within the first two weeks of the winter semester.

A student can apply for a study leave because of an illness, some unfortunate circumstances, child birth,  a need to attend care of a child or other justified reasons. You need to attach related documents or certificates to your application. You need to hand in the application right after the situation occurs.

The completed leave card in the original should be returned to the Student Service Center (COS) immediately, after obtaining a positive decision on granting a study leave from classes with the possibility of verification of achieved learning effects.

Not later than 7 days before the end of the study leave, the student is obliged to report his return to studies. Failure to meet the indicated deadline is tantamount to not taking up studies.

If you are a final year student and you have passed all the exams but you need a little bit more time to work on your Bachelor or Master thesis, you can apply for it using this application. You need to hand it in by the end of the re-take exam session of your last semester. Before you hand in the application in the Students’ Office, you need to get your thesis supervisor’s approval.

You can get no more than one month of extension.

If you happen to lose, have your Student Card stolen, or simply some of your most important personal data change (e.g. your surname) - use this application to apply for a new one.
